Your Home We can help you live in your own home or a shared community home. We call this support Person Centred working. Its “Your Home ,Your way, Your choice." Your Way
You tell us what support you want and what kind of person you would like to support you. You can talk to any manager about anything. We encourage you to be involved in the running of your service. Your Choice We will plan your support with you. We will help you to choose what you want to do in your home or out in the community. We call this person-centred planning. |
Supported Services
We are committed to supporting adults with support needs. We support individuals with learning and physical disabilities, autism, mental health needs, drug and alcohol dependency and acquired brain injuries to make their way along their own personal care pathway. Central to our support is enabling individuals to achieve their goals and aspirations.
We provide transitional and supported living services in Rotherham, Sheffield and Worksop to adults aged 18 and above. Our offer includes tailor made packages to enable individuals of all ages and abilities to live in their own home. Our Supported Living Services dovetail with our Transitional Services to support our clients in their transition into independent living in a safe way.
We seek to support our clients to access all facets of the community and promote and encourage independence. We work with partner organisations to encourage and support lifelong learning, supported employment and supported volunteering. We have an on-going commitment to developing independence and helping our clients achieve their personal outcomes.
People we support can also access the wider Horizon community and make use of the café, cinema and hydrotherapy rooms at a discounted rate.

The level of support is agreed after our assessment and is regularly reviewed and our aim to safety reduce our level of support to enable individuals to become more independent where possible.
